Please call or text the office to schedule a New Patient Appointment with Dr. Wise from our team, to see if he is the best fit for your needs. The New Patient visit usually lasts approximately 45 mins, and each subsequent visit is about 15 mins. We value your time in the office, for that we ask that all new patients print off the New Patient Forms linked below and have them filled out before you come in for your appointment. If its not possible to do so, we recommend coming in at least 15 mins before your appointment time to fill the forms out in the office.
